Why People Go to the Fitness Center : Agreement to Participate

We are inviting you to participate in a study about why people go to a fitness center by filling out a brief questionnaire. This will take approximately 10 minutes.

Your participation in this study is completely anonymous. Your participation is entirely voluntary and you may refuse to participate in the study or refuse to answer any questions you choose without penalty. There are no risks anticipated due to participation in this study, nor are there any benefits.
